I had a check up with this health provider (which shall remain nameless for the time being). It's been a while since I've had pre-employment medical checks so I found myself laughing at these questions on the form. (Click on photos to enlarge)

If I didn't have any mental complaint then, I sure as hell had one now...

What exactly does this mean? Are they implying that I've had gender reassignment and did a Chastity Bono or is it more Alexis Arquette?

And this is so politically incorrect.... Are we talking about general lameness like being totally uncool or is it just like geekiness or nerdiness?
